My Lord Inquisitor, herein follows a true and accurate description of the mission performed by your most obedient acolyte cell on the planet Sepheris Secundus.
We were ordered to investigate the loss of multiple Imperial Guard squads who had been sweeping the Gorgonid Mines for rebel holdouts. Our group was delivered by your pilot to the Imperial Guard encampment outside the mines. Let it be noted at this point that the craft in which we traveled was not in good condition and may be in need of comprehensive maintenance.
Once at the camp, our cell made contact with the commander of the guard forces, Commissar Nihilius. One of the rebels captured during the fighting was carrying a sheaf of papers. These papers described heresies most vile and were accompanied by whisperings of mutants, demons and worse. A loyal servant of the Emperor, Nihilius burned the papers and gathered a task force to cleanse the mines of heretic taint. The mission encountered trouble and only a few survivors returned to the surface. The commissar sealed the mine and contacted the inquisition, the result being our assignment to this mission. Commissar Nihilius should be congratulated for his quick thinking and unswerving dedication to the imperial cause.
After receiving additional equipment from the quartermaster, we passed through the seal and entered the mines. The upper levels were devoid of anything suspicious. As we neared the section known as 'The Shatters', we encountered a room filled with the viscera of a number of unholy mutants. Something had torn them apart. On our guard for further heresies, we descended into the shatters. As we stepped foot into this section of the mines, we found the floor to be covered in the rotting viscera of a multitude of dead bodies. Hardening our resolve, our group continued on into this deeply disturbing environment.
The first horror we came across was a guardsman, warped by the unholy powers into a form of cannibalistic ghoul. We destroyed the abomination in short order. More mutants were encountered and dispatched in a similar manner. It is worthy of noting that one of the mutants was not a shambling twisted wretch like the others. Rather it possessed long arms equipped with hooked talons. After some difficulty, this abomination joined its brothers in destruction.
Passing through a locked door into the deepest part of the mine, our cell came across a glowing lavender stone which filled the rough hewn rock chamber with a pinkish glow. This stone had, it seemed, been recently uncovered by the miners and reeked of the taint of the warp. With a sickly squeezing sound, a demon tore its way into reality and advanced on us. The demon was blasted by grenades before being cut down in a hail of gunfire. Celebrations were put on hold as it once again manifested itself through the eldritch powers of the stone. Further application of grenades destroyed the crystalline structure and the demon, losing its tether to real-space collapsed back into the warp.
Of the stone, only a shard remained. The shard was still tainted with unholy energies so, mindful of the havoc it could wreak if left unattended, we carefully sealed it in a solid metal box, packed with earth. We returned to the surface and recommended to Commissar Nihilius that the mines be cleansed with promethium and sanctified before being put back into operation. He agreed and we left the mines in his care.

Congratulations on your first successful mission in service of the Emperor's Inquisition. I am pleased by your accomplishment. Your actions have spoken well, and I have no reservations regarding your elevation to the formal rank of Acolyte of His Holiest Inquisition.
For now, as you are inducted, I will have a few small jobs for you to perform on Scintilla. I will be leaving shortly, so do not expect my presence for much longer - I expect that you all will be able to fend for yourselves with no issues.
Keep your faith strong and your bodies and minds pure, and you will continue to serve the Emperor well.
